How you can send your name to space

(CNN) -- If you're always wanted to leave your mark in space, here's your chance!

NASA is taking names into orbit.

The agency wants to put your name on a dime-sized microchip aboard an Orion Spacecraft that launches on December 4.

The test flight will circle earth a few times, before splashing down in the Pacific Ocean.

All you have to do is enter your name and a few other details on NASA's website, and you're ready to go the distance.

The names will also fly on future NASA exploration flights and missions to Mars.

Each name submitted receives a personal "boarding pass."

And with each flight, the selected names will accrue frequent flyer points.

The deadline to enter for the first test flight is October 31.
